Why does prison rape still occur?

The prison rape elimination act was passed in 2003. Despite this, it's still estimated that more men are raped each year than women. Why isn't anything done about this apparent epidemic of institutionalized torture? All I ever hear about it are sick jokes. I never hear anything about the actual problem being addressed.
